Turning to the specific methods of anesthesia provided in our Clinic, let's start with the most labor-intensive and science-intensive type of general anesthesia - combined, multicomponent endotracheal anesthesia. This type of anesthesia is most often used for long, voluminous surgeries that require complete relaxation of skeletal muscles, the patient in a forced position, blood pressure management, depth and frequency of respiration, a powerful analgesic effect.
The name of this type of anesthesia speaks for itself. During general anesthesia by this method, several groups of different drugs are used with different delivery routes and program points, taking into account their interaction in the human body, the peculiarities of excretion, effects on other organs and systems. Common in various variants of this technique is that during the operation the patient is in a state of deep sleep, breathing is provided by a ventilator and the main agent that supports this condition is "blown" into the patient's lungs. This main agent is drugs from the so-called group of inhaled anesthetics. In our clinic we use the inhalation anesthetic Sevoflurane. This is a modern anesthetic, developed from the experience of using similar drugs of previous generations and devoid of many of their shortcomings.
Thus, this drug does not accumulate in the human body and does not enter into its metabolic processes; after its supply at the final stage of the operation, it is completely removed with the air exhaled from the lungs, for a short period of time. It can be used in both newborns and elderly patients who already have heart disease. It is one of the few drugs for inhalation anesthesia that is used during cardiac surgery.
For accurate and safe dosing of this drug requires special, high-tech equipment that allows you to enter it in strictly calculated quantities. We use the latest generation anesthesia system "Flow-c". In addition to the above points, this device allows you to control at any time the concentration of inhaled and exhaled by the patient a mixture of sevoflurane and other medical gases (nitrous oxide), as well as monitors the concentration of oxygen and carbon dioxide, thus ensuring safe anesthesia. .
This device makes it possible to set such breathing parameters that they would be suitable for any patient, taking into account his age, anatomical features, underlying and comorbidities. In addition to the concentration of gases, the device "Flow-c" monitors a number of other parameters of respiration, immediately signaling any changes in their values, which allows the anesthesiologist without wasting a second to correct them and thus ensure maximum safety of anesthesia. . For the purpose of muscle relaxation (relaxation of skeletal muscles), one of the newest drugs - Esmeron is used in our clinic. This drug is unique in its ability, depending on the dose administered, to combine the properties of both short-acting and long-acting relaxants, without its accumulation in the body. And features of its metabolism in a human body allow not to be afraid of repeated relaxation (recurrence) after the operation.
The main type of anesthesia for small surgeries and diagnostic procedures is total intravenous anesthesia. In this case, the drug for anesthesia is administered intravenously, and the patient, although in a state of deep medical sleep, but continues to breathe alone, or with short-term support by a respirator through a face mask.
In order to quickly, safely enter into anesthesia, easily manage it at the required level and quickly get out of it at the end of the operation (manipulation), the doctors of our Clinic use the drug Propofol. This modern drug has proven itself in use in clinics in advanced countries. It has such important characteristics as safety, controllability, high speed of withdrawal that allows to use it even in an outpatient setting.
To ensure the safety and comfort of outpatient manipulations (esophagoduadenogastroscopy, colonoscopy, as well as minor gynecological interventions), a monitor with automatic measurement of blood pressure, SPO2, heart rate, temperature sensor is used. At the end of the manipulation, the patient is transferred under the supervision of medical staff to the ward of the hospital or day hospital (at the request of the patient).
In the presence of medical indications and (or) the patient's desire, a number of surgical interventions are performed using the methods of conduction anesthesia (spinal anesthesia, epidural, caudal, stem anesthesia). In this case, the main active ingredient is a local anesthetic, which is injected either directly into the nerve, the spinal cord, or under the dura mater, thereby turning off the sensitivity in the area of surgery. To create emotional comfort for the patient during surgery under conduction anesthesia, we additionally use small doses of sedatives or tranquilizers, thus achieving a state close to physiological sleep.
